Clinical Social Worker

We are seeking a skilled Clinical Social Worker to join our team at UMass Memorial Health Care. As a Clinical Social Worker, you will play a vital role in providing high-quality patient care and support to our patients and their families.
Job Summary:The Clinical Social Worker will work collaboratively with our interdisciplinary treatment team to develop and implement treatment plans for patients with complex medical and psychosocial needs. This includes conducting intake interviews, developing psychosocial evaluations, and coordinating discharge planning. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
Responsibilities:- Conduct intake interviews with patients and families to develop psychosocial evaluations
- Participate in team planning of treatment during initial interview and family meetings for goal setting and care planning
- Coordinate discharge planning for own case load and play a key role in case management
- Collaborate with outpatient providers to coordinate treatment and discharge planning
- Provide liaison function between palliative care team and social service, care coordination, and patient representative departments
- Attend weekly interdisciplinary team meetings and contribute to cases and teaching
- Graduation and training from an accredited graduate program with a current unrestricted licensure by the Massachusetts Board of Registration: MA/MSW, PhD (L.I.C.S.W/L.M.H.C/L.M.F.T.)
- Minimum of 2 years clinical experience in hospital, long-term care, or hospice setting
- Possess or actively work toward Palliative Care certification by National Association of Social Workers upon hire
