Job Summary

We are seeking a highly skilled Clinical Care Manager to join our team at South Middlesex Opportu. As a Clinical Care Manager, you will be responsible for providing direct care and support to our clients, as well as supervising a team of Care Coordinators.

Key Responsibilities
  1. Supervise and Train Care Coordinators: Hire, train, and supervise a team of Care Coordinators to ensure they have the necessary skills and knowledge to provide high-quality care to our clients.
  2. Lead a High-Performance Team: Motivate and lead a high-performance team of Care Coordinators, providing supervision, mentoring, and professional development opportunities to ensure they are equipped to meet the needs of our clients.
  3. Implement Quality Outcomes: Collaborate with team members to implement measures that decrease episodic care and meet quality outcomes.
  4. Provide Direct Care and Support: Serve as the Clinical Care Manager for assigned clients, providing direct care and support, and supervising Care Coordinators in doing the same.
  5. Engage Clients in Care: Utilize effective, dignified, empowering, and creative engagement strategies to ensure clients are at the center and lead in their care services.
  6. Conduct Outreach and Engagement: Conduct outreach and engagement activities with assigned clients, engaging them in enrolling in our program, and supervising Care Coordinators in providing this support.
  7. Assess and Develop Treatment Plans: Conduct comprehensive assessments in a collaborative manner with clients, with input from Care Team members and other stakeholders, and develop person-centered treatment plans.
  8. Collaborate with Care Team Members: Collaborate with existing providers, Care Team members, state agency staff, and all other stakeholders to deliver care services in accordance with client treatment plans.
  9. Participate in Team Meetings: Participate in BHCP team meetings and each client's Care Team with the assigned Care Coordinator when needed to ensure effective communication among all disciplines and stakeholders involved in the client's care.
  10. Identify Community Resources: Identify community resources and develop natural supports for clients, and supervise Care Coordinators in performing these functions.
  11. Complete Required Documentation: Complete all required documentation in a timely manner.
  12. Attend Supervision and Staff Meetings: Attend and actively participate in supervision and staff meetings.
  13. Consult with RNs and Other Care Team Members: Consult with RNs and other Care Team members as needed around clinical, medical, and other matters.
  14. Provide On-Call Coverage: Provide on-call coverage, as needed.
  15. Ensure Client Dignity and Respect: Ensure that all clients are treated with dignity and respect in accordance with BHPMW's Human Rights policy.
  16. Perform All Duties in Accordance with Agency Policies and Procedures: Perform all duties in accordance with the agency's policies and procedures.

Qualifications

We are seeking a candidate with a Master's degree with a first-level license (RN, LICSW, LMFT, LMHC, licensed psychologies, or LADAC I) with a minimum of 2 years of experience working in a community-based mental health, substance use, and/or physical health setting. A minimum of 2 years' supervisory experience is preferred. The ideal candidate will have demonstrated knowledge base and judgment necessary for independent clinical decision making, as well as strong skills in communication, follow-through, collaboration, and customer service.

We strongly prefer a candidate with a demonstrated understanding of and competence in serving culturally diverse populations. Excellent interpersonal skills and the ability to interact professionally with culturally and educationally diverse staff and patients are essential. Creativity, flexibility, sound judgment, and the ability to take initiative are also required. Excellent time management and organizational skills, as well as strong computer skills, including proficiency in contemporary Windows operating systems and Windows office suites, are necessary.

The ideal candidate will have a strong understanding of psychiatric rehab and the recovery model, as well as knowledge regarding evidence-based practices identified by the BHC. The ability to read and speak English is required, and a valid driver's license is necessary. The ability to spend 50% of time traveling and meeting with clients in their homes or in their communities is also required.
