Key Responsibilities:- Provide comprehensive, coordinated care to a mostly Medicare-aged population in various clinics, home, telemedicine, and mobile visits
- Collaborate with employed, contracted, and affiliated WellMed physicians, advance practice clinicians, and clinic staff to deliver high-quality patient care
- Educate providers on proper coding, risk adjustment metrics, capturing documentation, and adhering to quality protocols
- Interview patients to ensure proper documentation and preventative screenings
- Track and report performance metrics related to the organization's strategic direction and operational needs
