Overview
The Veterans Program Coordinator operates at the GS-12 level and holds administrative responsibility for the Spinal Cord Injury and Disorder (SCI) program within the Veterans Affairs system.

The SCI program delivers comprehensive care for all eligible Veterans who have experienced spinal cord injuries or have stable neurologic impairments related to the spinal cord.

Key Responsibilities
The incumbent will:

  • Direct Patient Care: Provide essential patient care services within the Patient Aligned Care Team (PACT) model, specifically for Veterans with spinal cord injuries.
  • Program Management: Oversee daily operations of the SCI program, including the development of policies and procedures, and the preparation of reports and statistics for various stakeholders.
  • Data Management: Manage and analyze program-related data to ensure effective functioning and evaluation of services.
  • Compliance: Ensure adherence to VHA directives and clinical practice guidelines in the delivery of care.

Additional Duties
Responsibilities include, but are not limited to:

  • Providing case management and supportive services to Veterans in need of SCI services, particularly during psychosocial crises.
  • Collaborating with clinic staff and other medical personnel to deliver comprehensive health care services, ensuring equitable access and benefits.
  • Collecting and analyzing data to monitor the quality of care provided.
  • Implementing treatment program policies that respect patient rights and maintain thorough documentation of patient progress.
  • Identifying SCI patients through various data systems and community outreach.
  • Coordinating annual health evaluations for enrolled SCI patients.
  • Preparing reports for various stakeholders, including the SCI Center and Congress.
  • Facilitating referrals and transfers between facilities as needed.
  • Establishing performance standards to enhance service quality and efficiency.
  • Exploring long-term care options for Veterans, including assisted living and nursing facilities.
  • Conducting psychosocial evaluations and developing treatment plans in collaboration with interdisciplinary teams.
  • Advocating for Veterans to ensure timely access to services and benefits.
  • Managing a range of services including personal care, nutritional management, and vocational rehabilitation.
  • Conducting home visits to assess living conditions and ensure safety and independence for Veterans.

Work Schedule
Monday to Friday, 8:00 AM to 4:30 PM. This position is not eligible for telework.

Requirements
Applicants must be U.S. citizens and meet specific licensure and educational qualifications, including a master's degree in social work from an accredited institution.

Preferred Experience
Two years of medical patient experience as a social worker is preferred.

Physical Requirements
Refer to VA Directive and Handbook 5019 for details on occupational health services.

Additional Information
Participation in vaccination programs for seasonal influenza and COVID-19 is required for all health care personnel.
