Job Title: Care Coordinator

We are seeking a highly motivated and organized Care Coordinator to join our team at Yavapai County, AZ. As a Care Coordinator, you will play a critical role in coordinating medical services and follow-up for patients at our Community Health Center of Yavapai (CHCY) locations.

Key Responsibilities:
  • Work cooperatively with patients and practitioners to identify problems and develop action plans.
  • Follow up on no-show appointments and facilitate patient referrals to specialty care.
  • Provide patient education on the importance of following through on prescribed treatments.
  • Assist with coordinating local marketing initiatives and community events.
  • Perform other job-related duties as assigned.
Requirements:
  • High school diploma or equivalent.
  • No experience required.
  • Valid Arizona driver's license.
  • Current fingerprint clearance card issued by the Arizona Department of Public Safety.
Preferred Qualifications:
  • Bilingual: Spanish/English speaking preferred.
