Overview:

The Front Door Diversion initiative is a state-funded program aimed at facilitating the transition of individuals from institutional settings back into the community, thereby preventing unnecessary nursing home placements. This program provides a comprehensive range of services, including community mental health support through an integrated healthcare approach and long-term housing assistance via transitional programs or subsidy services.

Role Summary:

The Housing Program Coordinator will oversee housing initiatives for the Front Door Diversion team, focusing on building and sustaining partnerships with landlords and private property management firms to assist our clients. This role involves collaboration with various teams and representatives from governmental bodies to ensure the availability of safe and affordable housing options for clients. Additionally, the Housing Program Coordinator will manage all components of the Bridge Subsidy Program in conjunction with Catholic Charities and the Statewide Referral Network (SRN), serving as the Agency Technical Administrator.

Key Responsibilities:

  • Establish and nurture relationships with landlords and property management companies to secure safe, affordable housing for clients, including partnerships with Catholic Charities and other organizations.
  • Oversee all housing-related activities for the Front Door Diversion program, including identifying eligible clients, locating housing units, and ensuring clients maintain housing in line with the housing-first model.
  • Provide direct support to clients regarding housing matters, including lease negotiations, move-in processes, inspection requests, and completion of necessary housing documentation.
  • Collaborate with team members to enhance client care and service delivery, while actively seeking new housing opportunities.
  • Educate and support team members on housing-related issues to facilitate client housing security, including conducting training sessions as needed.
  • Manage the dynamics between tenants, clients, and the team to ensure effective communication and support.
  • Assist clients in maintaining their participation in the Catholic Charities Bridge Subsidy and facilitate transitions to the Chicago Housing Authority (CHA) when necessary.
  • Compile and report relevant information to state and local authorities regarding program performance and compliance.
  • Serve as a trained Skilled Assessor capable of enrolling homeless clients in the Coordinated Entry System (CES) and Statewide Referral Network (SRN) for emergency housing assistance.
  • Perform additional duties and projects as assigned.

Qualifications:

  • One of the following educational backgrounds is required:
    • Bachelor's degree in counseling, social work, psychology, or a related field.
    • Bachelor's degree in any field with two years of supervised clinical experience in a mental health environment.
    • Licensed Practical Nurse under the Nurse Practice Act.
    • Certificate of Psychiatric Rehabilitation from a recognized program, along with a high school diploma/GED and two years of experience in a mental health setting.
    • CRSS certification or Family Partnership Professional certificate in good standing.
    • Occupational Therapy Assistant licensed under the Illinois Occupational Therapy Practice Act with one year of mental health experience.
    • High school diploma or GED with five years of supervised clinical experience in a mental health setting.
  • Possession of a valid Illinois driver's license and access to a reliable vehicle with appropriate liability insurance.
  • Preferred experience working with individuals facing mental health and/or substance use challenges.
  • Preferred background in community engagement, advocacy, and marketing.
