Compensation:

This role involves assisting clients with:
- Personal care requirements
- Companionship and social engagement
- Meal preparation
- Light housekeeping tasks
- Shopping and errands
- Medication reminders
- Wellness monitoring

Requirements:
- Certified Home Health Aide/Certified Nurse Aide, Personal Care Attendant, or relevant home care experience
- Reliable transportation
- Punctuality and dependability
- Ability to utilize a smartphone-based application

- Facilitating smooth transitions home from hospitals or rehabilitation centers following surgeries
- Ensuring clear and timely communication with families and other healthcare providers through real-time access to care notes and updates from our Care Advocates
- Supporting families in caring for their loved ones while managing all details related to hospital admissions and ensuring caregivers are specifically matched to meet clients' unique needs.

In addition to our monthly in-service training, we provide advanced training to our caregivers, equipping them to identify and escalate concerns related to common causes for hospital visits.
