Job Title: Registered Nurse RN Home Health

Job Summary:

Banner Health is seeking a skilled Registered Nurse RN to join our Home Health team. As a Home Health RN, you will provide exceptional care to patients in their homes, focusing on medical intervention and creating one-on-one relationships with patients and their caregivers. This role offers a flexible schedule, with options for 3 12hr shifts, 3 10hr shifts, or 3 8hr shifts, with a commitment to working every other weekend or every weekend with flexibility to work one weekday.

Responsibilities:

  1. Assess patient physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs.
  2. Formulate a plan of care, including the discharge plan, utilizing assessment data and patient, family, and health team input.
  3. Implement the plan of care through direct patient care, coordination, delegation, and supervision of the activities of the health care team.
  4. Evaluate the patient's and family's response to care and teaching, and revise the plan of care as needed.
  5. Document assessment, planning, implementation, and evaluation in the patient record.

Requirements:

  • Must possess a current, valid RN license in the state of practice.
  • BLS certification is required.
  • Additional certification or continuing education may be required based on area of practice.
