About the Role

We are seeking a skilled and compassionate Home Health Registered Nurse to join our team. As a Home Health RN, you will have the opportunity to provide one-on-one care to patients in the comfort of their own homes. You will work closely with our patients to develop personalized care plans, assess their condition, and provide skilled interventions as ordered.

Responsibilities
  • Conduct home visits to assess patient condition and progress toward goals
  • Develop and implement care plans in collaboration with Case Managers
  • Provide patient education and support to promote independence and well-being
  • Collaborate with healthcare teams to ensure seamless care transitions
  • Document patient progress and outcomes in accordance with company policies
Requirements
  • Current Registered Nurse (RN) license in the state of Arizona, in good standing
  • CPR Certification (demonstration course required)
  • Valid Arizona Fingerprint Card
  • One (1) year of home health experience preferred but not required
