Home Health RN Opportunity

**About the Role**

We are seeking a skilled Home Health Registered Nurse to join our team at Interim HealthCare. As a Home Health RN, you will provide high-quality patient care in the comfort of their own homes. Our team is dedicated to delivering exceptional care and support to our patients, and we are looking for a compassionate and experienced nurse to join us.

**Responsibilities**

* Complete initial and ongoing comprehensive assessments of patients
* Coordinate patient discharge from services
* Provide care according to the patient's plan of care
* Participate with patients in clarifying mutually agreed upon goals and incorporate these goals into the plan of care
* Conduct home visits as assigned, assessing the patient's condition, progress toward goals, providing patient care, teaching and other skilled interventions as ordered
* Develop a care plan in collaboration with Case Manager, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions

**Requirements**

* Current Registered Nurse (RN) license in the state of Arizona, in good standing
* CPR Certification (demonstration course required)
* Valid Arizona Fingerprint Card
* One (1) Home health experience preferred but not required
