Home Health Registered Nurse Opportunity

We are seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Registered Nurse. As a Home Health Registered Nurse, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to develop and implement personalized care plans.
Key Responsibilities- Assess and Evaluate Patient Needs: Conduct comprehensive assessments of patients to identify their physical, emotional, and social needs, and develop care plans to address these needs.
- Provide Skilled Nursing Care: Provide skilled nursing care to patients in their homes, including wound care, medication management, and education on self-care.
- Collaborate with Interdisciplinary Team: Work closely with our interdisciplinary team, including physicians, therapists, and social workers, to develop and implement comprehensive care plans.
- Communicate Effectively: Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination and patient satisfaction.
- Document Patient Care: Accurately and thoroughly document patient care, including assessments, interventions, and outcomes, to ensure continuity of care and compliance with regulatory requirements.
- Current RN License: Current Registered Nurse (RN) license in the state of Arizona, in good standing.
- CPR Certification: CPR Certification (demonstration course required).
- Valid Arizona Fingerprint Card: Valid Arizona Fingerprint Card.
- Home Health Experience: One (1) year of home health experience preferred but not required.
