Position Overview
We are seeking a full-time Nurse Practitioner to provide high-quality medical services at our partner Nursing and Rehabilitation Centers. This role is essential in supporting our mission to enhance clinical excellence and outstanding service in the facilities we collaborate with.
Key Responsibilities
- Conduct comprehensive history and physical examinations for patients.
- Manage patient care through admissions, rounds, and discharges in skilled nursing facilities.
- Collaborate with a multidisciplinary team, including Medical Directors and Attending Physicians, to ensure optimal patient outcomes.
- Provide evaluations for patients experiencing changes in their medical condition.
- Maintain compliance with all quality initiatives set forth by Five Star Physician Services and partner facilities.
Work Schedule and Environment
This position offers a Monday through Friday schedule with flexible hours, allowing for a work-life balance while ensuring that you can focus on patient care. Our supportive environment includes extensive training and quality assurance feedback to help you excel in your role.
