Position Summary:

The primary objective of the Nurse Manager role is to engage in and supervise the delivery of patient care as directed by the attending physician, adhering to established standards of practice. This role entails providing direct care to patients and overseeing the care provided to ensure optimal patient outcomes.

Qualifications:
  • Completion of an accredited Nursing Program and possession of a valid LVN/LPN license in the relevant State.
  • A minimum of one year of supervisory experience in a long-term care or healthcare environment is preferred.
  • Proficiency in reading, writing, and speaking the English language.
Key Responsibilities:
  • Conduct daily reviews to ensure accuracy, completeness, and appropriate interventions in:
    • Telephone Orders
    • Medication Administration Records
    • PRN Medication Administration Records
    • IV Medication Administration Records
    • Treatment Records
    • G-Tube Records
    • Diabetic Records
    • Anticoagulant Monitoring Forms
    • Weekly Skin Assessment Forms
    • Patient Daily Records
    • Flow Records
    • Behavior Monitoring Sheets
    • Narcotic Control Sheets
    • Monthly Summaries
    • Lab and X-ray reports
    • Glucometer Control Logs
    • Refrigerator Temperature Logs
  • Perform rounds on all patients at least every four hours and as necessary, communicating any concerns to the charge nurse.
  • Review the 24-hour report book for updates on patient conditions, ensuring any changes are assessed, documented, and addressed.
  • Evaluate incident reports to ensure proper documentation of nursing assessments and interventions.
  • Verify the accuracy of medical records for discharged patients and ensure proper follow-up documentation.
  • Check documentation regarding patient room changes and admissions.
  • Conduct follow-up reviews of medical records to identify problems and ensure timely interventions.
  • Distribute the monthly MDS/care plan calendar to the interdisciplinary team by the 15th of each month.
  • Complete MDS assessments, RAPs if applicable, Care Plans, and TILE assessments as required.
  • Participate in weekly care plan meetings with the interdisciplinary team, patients, and responsible parties.
  • Attend weekly TILE and Medicare meetings with relevant team members.
  • Input physician orders daily into the clinical software system.
  • Print and verify physician orders for accuracy by the 25th of each month.
  • Address recommendations from the Consultant Pharmacists and Dietary Consultant Reports within the specified timeframe.
  • Ensure emergency medication boxes are properly maintained.
  • Document patient weights in the clinical software system as per guidelines.
  • Update and distribute Daily Care Guides weekly.
  • Review Cantex Continuing Care Networks Clinical Policies and Procedures for Abuse Prevention and understand employee responsibilities.
  • Attend weekly in-services and comply with Patient Care Management Systems.
  • Ensure patient safety is prioritized at all times.
  • Perform additional duties as assigned.
