Job Summary

Sea Mar Community Health Centers is seeking a highly motivated and compassionate Case Manager to join our Care Management Department in Bellingham, WA. As a key member of our team, you will provide support to patients and families, and technical or caseload support to the care management team. Your primary responsibility will be to assist patients in achieving their self-management goals, improving their healthcare experience, and enhancing their overall well-being.

Key Responsibilities
  • Meet with patients in their homes, communities, or clinics to provide support and guidance.
  • Assist patients in setting and achieving small steps towards their goals, promoting skill building and empowerment.
  • Participate in groups such as Living Well with Chronic Conditions or Chronic Disease Self-Management Program.
  • Advocate for patients with chronic conditions, understanding the emotional and psychological aspects of these conditions.
  • Handle patient information and activities with confidentiality and professionalism.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements
  • High School Graduate or equivalent; Bachelor's degree preferred.
  • One year of paid or volunteer experience in medical, social service, or behavioral health case management or care coordination.
  • Background in community outreach or case management is preferred.
  • Attended or plan to attend WA State Community Health Worker Training and/or Chronic Disease Self-Management training.
  • CPR certification or obtain it within the initial probationary period.
  • Valid driver's license and proof of insurance.
  • Ability to write routine reports and correspondence, and speak effectively before groups.
  • Bilingual in English/Spanish is preferred but not required.
