Case Manager

The Case Manager position at CHRIS 180 is a vital role in supporting the rehabilitation, environmental support, guidance, and resources coordination for youth and young adults. As a Case Manager, you will play a crucial part in assisting individuals in gaining access to necessary services and creating environments that promote resiliency and support emotional and functional growth and development.
Key Responsibilities:
- Assist with youth referrals from various organizations, including Atlanta Public Schools, Fulton County Schools, Department of Juvenile Justice, Probation Office, Atlanta Police Departments, and Fulton County Courts.
- Display positive role modeling to members, promote self-esteem, healthy boundaries, good hygiene, and appropriate personal appearance.
- Reinforce socially acceptable behaviors, teach and reinforce thoughtful decision-making, professionalism in work tasks, and employment.
- Assess members' history of trauma, strengths, and needs, and develop and monitor Individualized Service Plans.
- Coordinate services and collaboration with other programs in the CHRIS 180 continuum, other providers, and entities to provide best care services to members.
- Transport members to and from facilities and community sites, meet with assigned members monthly, and monitor the provision of services.
Requirements:
- Experience working with youth and transition-age young adults with mental health and/or substance abuse diagnoses.
- Bachelor's Degree in a field of social services, such as Psychology, Social Work, or Sociology.
- Ability to effectively relate to consumers, parents/custodians, and staff from diverse backgrounds, with firmness, fairness, and concern.
- Solid decision-making ability related to the safety and well-being of consumers, families, community, and staff.
- Dependability, emotional stability, and physical capability.
Working Conditions:
This position requires some physical functions, such as sitting, standing, walking, driving, speaking, and hearing. The ability to physically restrain when necessary is also required. The ideal candidate will possess technical expertise, be accountable for work completion, and have the ability to collaborate with other staff to ensure best practices and sound decisions.
