Case Manager RN

CVS Health is seeking a skilled Case Manager RN to join our team. As a Case Manager RN, you will play a critical role in delivering enhanced human-centric healthcare to our patients. You will work collaboratively with our multidisciplinary team to assess, plan, facilitate, coordinate, evaluate, and advocate for options and services to meet the comprehensive health needs of our patients.
Key Responsibilities- Act as a liaison with patients, families, employers, providers, insurance companies, and healthcare personnel to ensure seamless care coordination.
- Implement and coordinate case management activities for catastrophic cases and chronically ill patients, including consultant referrals, home care visits, and alternative levels of care.
- Assess and analyze patients' medical and/or vocational status to develop a plan of care that optimizes wellness and medical outcomes.
- Communicate effectively with patients, families, and stakeholders to ensure timely and effective care coordination.
- Prepare and maintain accurate documentation of case work activities.
- Collaborate with internal multidisciplinary teams to ensure best health outcomes for patients.
- Active and good standing Compact RN license.
- Minimum 3+ years of clinical experience, preferably in diabetes, CHF, CKD, post-acute care, hospice, palliative care, or cardiac care.
- Ability to travel occasionally for in-person case management activities.
- Minimum 2+ years of case management, discharge planning, and/or home health care coordination experience.
- Excellent analytical and problem-solving skills.
- Effective communication, organizational, and interpersonal skills.
- Ability to work independently and navigate multiple systems.
- Associate's degree or Nursing Diploma required.
- Bachelor's degree preferred.
The typical pay range for this role is $54,116.80. This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors, including experience, education, geography, and other relevant factors.
