Job Summary:

The Bilingual Case Manager will be responsible for helping children and families with the reunification process by providing casework and supportive services. This includes communicating with and assessing family members identified or referred for potential sponsorship, and assisting with sponsorship applications as needed.

Key Responsibilities:
  • Ensure that all assigned youth on your caseload receive mandatory services, including mental health, medical, recreational, educational, and other supportive services.
  • Develop and complete assessments, service and treatment plans, progress notes, incident reports, and other necessary documentation in a timely manner.
  • Create a trusting and safe relationship with clients and encourage personal growth.
  • Maintain accurate and current information on children in care, ensuring ORR's, LIRS's, and the program's documentation practices and reporting requirements are met.
  • Actively participate in weekly staff meetings with the clinical team and stakeholders.
  • Attend weekly meetings with supervisor to consult on cases, review case plan decisions, and evaluate client participation and progress.
  • Maintain effective communication with community agencies or individuals involved in service provision.
  • Attend internal and external training and apply newly gained knowledge in providing services.
  • Transport clients as needed.
  • Explore potential community resources and refer UC and families as needed.
  • Maintain updated, working knowledge of and adhere to federal, state, program, and agency regulations, policy, and operating procedures.
  • Have the ability to work flexible hours and work independently within the field.
  • Provide translation support as needed for clients, sponsors, and staff.
  • Comply with laws and regulations regarding client confidentiality.
Requirements:
  • Bilingual in English and Spanish preferred.
  • Bachelor's degree in Human Services, Social Work, or related field of study from an accredited college.
  • At least one year of child welfare case management experience.
  • Ability to handle a maximum caseload of 8 clients.
  • Must be 21 years of age and pass a criminal history screen, including state and local child protection agency registries.
  • Excellent organizational and time management skills.
  • Ability to collaborate with others, including case management staff, educational team members, supervisors, and external stakeholders.
  • Ability to practice cultural sensitivity and work effectively with diverse populations.
  • Ability to be flexible with working hours and on-call responsibilities.
  • Excellent writing and communication skills.
  • Computer skills adequate to perform essential functions (using the UC Portal, Microsoft Office, and any internal case management software).
