Job Title: Case Manager

At Aniz Inc, we are seeking a highly skilled and compassionate Case Manager to join our team. As a Case Manager, you will play a critical role in ensuring the well-being and success of our clients.

Key Responsibilities:
  • Assess clients' treatment needs and develop personalized treatment plans
  • Monitor and evaluate treatment progress, making adjustments as needed
  • Facilitate interdisciplinary approaches to care, collaborating with healthcare professionals and other stakeholders
  • Admit new clients, reviewing records and applications, and conducting orientations
  • Determine clients' requirements through intake interviews and reviews of therapist evaluations
  • Establish treatment programs, setting schedules and routines, and coordinating services
  • Monitor cases, verifying attendance, observing treatments, and advocating for needed services
  • Maintain clients' records, reviewing case notes and logging events and progress
  • Communicate clients' progress through weekly interdisciplinary meetings and evaluations
  • Prepare clients for discharge, reviewing and amplifying discharge plans, and coordinating post-discharge requirements
Requirements:
  • Master's in social work preferred
  • 1+ year of experience in Rapid Rehousing programs
  • Professional or personal experience supporting individuals with disabilities, mental illnesses, or challenging behaviors
