Job Overview

Position Title: Team Leader

Reports To: Program Director

Position Summary:

Resources for Human Development (RHD) is seeking a dedicated Full-Time Team Leader to oversee the coordination of comprehensive services for individuals and their program supports in the Trumbull area. The Team Leader collaborates closely with program management, clinical staff, and direct support professionals to ensure that each individual is empowered to achieve their desired quality of life.

Essential Duties:

Provide support activities within homes to ensure individuals experience a high quality of life.

Actively engage in the person-centered planning process, which includes:

  • Attending all planning meetings.
  • Offering input and coordinating training plans.
  • Ensuring all necessary documentation is completed.
  • Reviewing progress and daily notes.
  • Monitoring the implementation of care plans.
  • Completing monthly reviews and communicating with case managers.
  • Coordinating medical appointments and communicating with healthcare professionals as needed.
  • Maintaining positive relationships with family members, guardians, staff, and regulatory personnel.
  • Preparing and submitting reports related to individuals supported.
  • Ensuring compliance with all regulatory requirements.
  • Implementing continuous quality improvement initiatives.

Qualifications:

  • Strong communication skills.
  • Excellent organizational and time-management abilities.
  • Basic computer literacy and proficiency in using applications for data entry and retrieval.
  • Must be certified by the State of Connecticut to administer medication.
  • Valid Driver's License.
  • Subject to employment screenings, including background checks.

Education Requirements:

  • Bachelor's Degree in a Human Services field (e.g., sociology, special education, rehabilitation, nursing, counseling, psychology) or
  • An Associate Degree with three years of experience supporting individuals with intellectual disabilities or
  • A High School Diploma with five years of relevant experience.

Physical Requirements:

  • Medium lifting capabilities (up to 50 pounds occasionally, 20 pounds frequently, and 10 pounds constantly).
  • Ability to stand, walk, use hands, climb, communicate, and see as necessary for the role.
