Population Health Coordinator Opportunity

Tapestry 360 Health is seeking a dedicated Population Health Coordinator to serve as a crucial link between patients, healthcare services, and community organizations, focusing on improving health outcomes at both individual and population levels.

Key Responsibilities:

  • Provide essential peer support and advocacy for patients, promoting health equity and helping others achieve their wellness goals.
  • Assess social determinants of health and connect patients with necessary resources, such as healthcare services, social services, and community organizations.
  • Coordinate with healthcare team members to enhance patient care delivery, ensuring seamless transitions between settings and effective communication with diverse populations.
  • Enroll and educate patients in disease management programs, providing them with the knowledge and skills necessary to manage their conditions and improve their health outcomes.
  • Facilitate transitions of care between settings, ensuring that patients receive consistent and high-quality care throughout their healthcare journey.
  • Support quality improvement efforts through targeted patient outreach and education, helping to identify areas for improvement and develop strategies to address them.

Requirements:

  • Minimum of two years of experience in healthcare or public health, with a strong understanding of healthcare systems and services.
  • Proficiency in electronic medical record systems and Microsoft Office, with the ability to effectively communicate and engage with diverse populations.
  • High school diploma or GED required, with a preference for associate's or bachelor's degree in a related field.

Preferred Qualifications:

  • Experience in hypertension, diabetes, and depression interventions, with a strong understanding of the social determinants of health and their impact on patient outcomes.
