About the Role

The Chicago Lighthouse is seeking a skilled Mental Health Care Coordinator to join our team. As a key member of our behavioral health initiative, you will play a critical role in coordinating services for clients with vision loss and other specialized support needs.

Key Responsibilities
  • Coordinate all aspects of behavioral healthcare services, including assessment, treatment planning, and service delivery.
  • Facilitate services for clients with vision loss and other specialized support needs, ensuring a comprehensive and coordinated approach to care.
  • Collaborate with multidisciplinary teams, including healthcare providers, social workers, and other support staff, to ensure seamless service delivery.
  • Develop and maintain relationships with community partners, including healthcare providers, social services agencies, and other organizations, to ensure access to resources and services.
  • Monitor and evaluate the effectiveness of behavioral health services, making recommendations for improvement and ensuring compliance with regulatory requirements.
Requirements
  • Bachelor's degree in a related field, such as social work, psychology, or counseling.
  • Experience in behavioral health services, including assessment, treatment planning, and service delivery.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations and stakeholders.
  • Ability to work in a fast-paced environment, prioritizing multiple tasks and deadlines.
  • Strong analytical and problem-solving skills, with the ability to think critically and make sound judgments.
