Key Responsibilities:
- Deliver direct support services, including one-on-one counseling, case management, advocacy, and accompaniment.
- Ensure compliance with case management standards by consistently monitoring client progress towards personal and programmatic objectives.
- Conduct initial client assessments: evaluate applications and engage with new clients through interviews.
- Input client data into the designated database within a specified timeframe.
- Develop individualized service plans for each client, prioritizing critical issues.
- Collaborate with service providers to facilitate the achievement of each client's service plan goals.
- Support clients in achieving stability by providing resources and connections to necessary services.
- Engage in problem-solving discussions with clients to identify the most effective support strategies.
- Observe clients to identify potential issues requiring intervention.
- Lead meetings and support groups as necessary.
- Create client exit surveys and case summaries to enhance program evaluation and outcomes.
- Maintain accurate electronic client records, case files, daily logs, and service statistics in a timely manner.
- Participate in regular team meetings, including case management discussions and program development sessions.
- Engage in ongoing professional development and training opportunities.
- Perform additional duties as assigned.
Qualifications:
- Bachelor's degree in social services, psychology, or a related field. Equivalent professional experience may be considered in lieu of a degree.
- A minimum of two years of experience in providing services to individuals experiencing chronic homelessness is preferred. Relevant experience will also be taken into account.
