We are seeking a highly skilled Primary Care Physician to join our team at CenterWell Senior Primary Care. As a Primary Care Physician, you will play a vital role in providing proactive, preventive care to seniors, including wellness visits, physical exams, chronic condition management, screenings, and minor injury treatment.
Key Responsibilities- Provide high-quality, patient-centered care to seniors, focusing on wellness, prevention, and disease management.
- Conduct thorough physical exams, diagnose and treat acute and chronic conditions, and develop personalized treatment plans.
- Manage chronic conditions, such as diabetes, hypertension, and heart disease, and provide education and support to patients and their families.
- Order and interpret diagnostic tests, such as lab work and imaging studies, and develop treatment plans based on test results.
- Collaborate with other healthcare professionals, including nurses, behavioral health specialists, and specialists, to provide comprehensive care to patients.
- Stay up-to-date with the latest medical research, guidelines, and best practices, and apply this knowledge to improve patient outcomes.
- Doctorate degree in medicine (MD) or osteopathic medicine (DO) from an accredited university.
- 5 or more years of experience in primary care, preferably in a value-based care or managed care setting.
- Licensure to practice medicine in the State of Florida.
- Board certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Bilingual (Spanish/English) speaking skills.
