As a Primary Care Physician at CenterWell Senior Primary Care, you will be part of a team that provides proactive, preventive care to seniors. Our unique care model focuses on personalized experiences, taking time to listen, learn and address the factors that impact patient well-being.
As a Physician Leader, you will be responsible for providing high-quality patient care, managing a team of healthcare professionals, and contributing to the development of our care model. You will have the opportunity to work with a diverse patient population, addressing their physical, emotional, and social wellness needs.
We are seeking a Physician with a strong background in outpatient medicine, who is passionate about delivering exceptional patient care and leading a team of healthcare professionals. Key Responsibilities- Provide high-quality patient care, managing a panel of patients and coordinating care with other healthcare professionals
- Lead a team of healthcare professionals, providing guidance and support to ensure excellent patient outcomes
- Contribute to the development of our care model, working with a multidisciplinary team to improve patient care and outcomes
- Participate in quality improvement initiatives, identifying areas for improvement and implementing changes to enhance patient care
- Doctorate degree in Medicine or a related field
- 5 or more years of experience in outpatient medicine, with a strong background in leadership and team management
- Licensure requirements in the State of Florida
- Graduate of an accredited MD or DO program
- Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine
- Bilingual (Spanish/English) speaking
