About the Role:
CenterWell Senior Primary Care is seeking a compassionate and skilled Primary Care Physician to join our team at West Vine. As a Primary Care Physician, you will play a vital role in providing proactive, preventive care to seniors, including wellness visits, physical exams, chronic condition management, screenings, and minor injury treatment.
Responsibilities:
- Provide high-quality, patient-centered care to seniors in a fast-paced, dynamic environment.
- Collaborate with our integrated care teams, including physicians, nurses, and behavioral health specialists, to deliver comprehensive care.
- Develop and implement personalized care plans to address the unique needs of each patient.
- Stay up-to-date with the latest medical research and best practices to ensure the highest level of care.
- Participate in quality improvement initiatives to enhance patient outcomes and experiences.
Requirements:
- Doctorate degree in medicine (M.D.) or osteopathic medicine (D.O.) from an accredited university.
- 5 or more years of experience in primary care, preferably in a value-based care or managed care setting.
- Licensure requirements in the State of Florida.
- Board certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
Preferred Qualifications:
- Bilingual (Spanish/English) speaking.
