Primary Care Physician

About the Role:
CenterWell Senior Primary Care is seeking a skilled Primary Care Physician to join our team in West Vine. As a Physician, you will play a vital role in providing proactive, preventive care to seniors, including wellness visits, physical exams, chronic condition management, screenings, and minor injury treatment.
Key Responsibilities:
- Provide high-quality, patient-centered care to seniors in our outpatient setting.
- Develop and implement individualized care plans to address the unique needs of each patient.
- Collaborate with our integrated care teams, including nurses, behavioral health specialists, and other healthcare professionals, to ensure comprehensive care.
- Stay up-to-date with the latest medical research and best practices to provide evidence-based care.
- Participate in quality improvement initiatives to enhance patient outcomes and experiences.
Requirements:
- Doctorate degree in medicine (MD) or osteopathic medicine (DO).
- 5 or more years of experience in Value-Based Care/Managed Care.
- Licensure requirements in the State of Florida.
- Graduate of an accredited MD or DO program of an accredited university.
- Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
Preferred Qualifications:
- Bilingual (Spanish/English) speaking.
