Physician - Primary Care Specialist

CenterWell Senior Primary Care is seeking a skilled Physician to join our team as a Primary Care Specialist. As a key member of our healthcare team, you will be responsible for providing high-quality, patient-centered care to our senior patients.
Key Responsibilities- Provide comprehensive medical care to patients, including wellness visits, physical exams, and chronic condition management.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Stay up-to-date with the latest medical research and best practices to provide evidence-based care.
- Doctorate degree in medicine (MD) or osteopathic medicine (DO).
- 5+ years of experience in primary care or a related field.
- Medical licensure in the state of Florida.
- Board certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Bilingual or multilingual skills.
- Experience working with seniors or vulnerable populations.
