Palliative Care Nurse Practitioner Position Summary
  • Deliver high-quality, patient-centered care that addresses the physical, emotional, and spiritual needs of patients with serious illnesses.
  • Work collaboratively with the palliative medical director and other healthcare team members to develop and implement individualized care plans.
  • Prescribe medications, order laboratory tests, and recommend therapies to manage symptoms and optimize quality of life.
  • Maintain accurate and timely documentation of patient care in electronic medical records.
  • Stay current with industry developments and best practices in palliative care through ongoing education and professional development opportunities.
Key Responsibilities
  • Assess patient needs and develop treatment plans that address physical, emotional, and spiritual distress.
  • Prescribe medications, order diagnostic tests, and provide education to patients and their families on disease management and symptom control.
  • Maintain effective relationships with patients, families, and other healthcare team members to ensure seamless coordination of care.
  • Stay informed about new treatments, technologies, and research findings in palliative care.
  • Collaborate with other healthcare professionals to achieve optimal patient outcomes.
