**Job Description:**

We are seeking a highly skilled Nurse Practitioner to join our team of compassionate caregivers. As a Hospice and Palliative Care Nurse Practitioner, you will provide exceptional patient-centered care to individuals facing life-limiting illnesses.

**Responsibilities:**

  • Provide comprehensive palliative care and hospice services to patients in their own homes or in healthcare facilities.
  • Collaborate with a multidisciplinary team to deliver high-quality care that aligns with each patient's goals and values.
  • Assess and manage symptoms, providing relief and support to patients and their families.

**Requirements:**

  • Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing.
  • At least 2 years of experience within the last 5 years in chronic disease management or long-term care, with a preference for hospice background.
  • Reliable transportation and ability to sit, stand, bend, move intermittently, and lift at least 80-100 lbs.
