Palliative Care Nurse Practitioner Job Description
  • Provide comprehensive and individualized care to patients with serious illnesses, working closely with the palliative medical director and other interdisciplinary team members.
  • Assess patient needs and develop treatment plans that address physical, emotional, and spiritual distress.
  • Prescribe medications, order diagnostic tests, and provide education to patients and their families on disease management and symptom control.
  • Maintain accurate and timely documentation of patient care in electronic medical records.
  • Participate in multidisciplinary meetings to coordinate care and share knowledge with colleagues.
Requirements and Qualifications
  • Master's degree in nursing from an accredited institution.
  • Current licensure as a registered nurse or advanced practice registered nurse (APRN).
  • Minimum 2 years of experience as a nurse practitioner in palliative care or a related field.
  • National certification in hospice and palliative nursing preferred.
  • Familiarity with electronic medical records and point-of-care software.
