Nurse Practitioner in Hospice and Palliative Medicine

We are seeking a highly skilled and compassionate Palliative Care Nurse Practitioner to join our team. As a member of our interdisciplinary team, you will play a critical role in delivering exceptional care to patients with serious illnesses.
Duties and Responsibilities- Provide high-quality, patient-centered care that addresses the physical, emotional, and spiritual needs of patients with serious illnesses.
- Work closely with the palliative medical director and other healthcare team members to develop and implement individualized care plans.
- Prescribe medications, order laboratory tests, and recommend therapies to manage symptoms and optimize quality of life.
- Maintain accurate and timely documentation of patient care in electronic medical records.
- Participate in ongoing education and professional development opportunities to stay current with best practices in palliative care.
