**Your Role:**

We're seeking a skilled Nurse Practitioner to join our team. In this role, you'll provide expert care to seriously ill patients, focusing on alleviating symptoms and aligning treatments with their values.

**Responsibilities:

  • Collaborate with a team of clinicians to deliver ongoing palliative care and hospice services
  • Provide education and support to patients and families in various home settings
  • Participate in the development of individualized care plans to meet the unique needs of each patient
  • Work closely with other healthcare professionals to ensure seamless transitions and comprehensive care

**Requirements:

To be successful in this role, you'll need:

  • A graduate degree in nursing (MSN or DNP)
  • 2+ years of experience in chronic disease management or long-term care, preferably in hospice
  • Excellent communication and interpersonal skills
  • Ability to sit, stand, bend, move intermittently, and lift at least 80-100 lbs
