Advanced Illness Care Specialist

As an Advanced Illness Care Specialist at Agape Care Group, you will play a crucial role in providing high-quality care to patients with advanced illnesses. Your expertise as a Hospice and Palliative Nurse Practitioner will be utilized to alleviate symptoms, align treatments with patients' values, and provide education and support in various home settings.
Responsibilities:
- Collaborate with a team of clinicians to deliver palliative care and hospice services
- Conduct patient assessments and develop individualized care plans
- Provide education and support to patients and their families
- Stay up-to-date with the latest developments in palliative care and hospice services
Requirements:
- Graduate of an accredited nursing program
- 2 years experience within the last 5 years in chronic disease management/long-term care
- Reliable transportation and ability to lift at least 80-100 lbs
