As a vital member of the WestCare Foundation team, the Care Coordinator plays a crucial role in supporting clients who have experienced trauma. This position involves advocating for clients, facilitating access to essential services, and ensuring seamless collaboration with various organizations to provide comprehensive care for children affected by violence and their non-abusing guardians.
Key ResponsibilitiesThe Care Coordinator is responsible for:
- Delivering direct support to children impacted by violence and their non-abusing parents.
- Coordinating services with partner agencies, including making necessary referrals.
- Conducting thorough Needs Assessments to identify client requirements.
- Preparing detailed service plans tailored to individual client needs.
- Collaborating with treatment teams to develop effective discharge plans.
- Facilitating case management sessions and Service Plan development.
- Maintaining accurate client records in compliance with regulatory standards.
- Participating in training sessions, team meetings, and community initiatives.
- Performing additional duties as assigned by supervisory staff.
- Upholding confidentiality and adhering to professional conduct standards.
Essential Qualifications:
- Certifications: Candidates should be eligible for CAP, CPRS, or CPMH certification. Preference will be given to those who are already certified.
- CPR/First Aid certification is required within six months of employment and must be renewed biennially.
Education:
- A bachelor's degree from an accredited institution in fields such as Addiction Counseling, Criminal Justice, Social Work, Psychology, or a related discipline is mandatory.
Experience and Skills:
- A minimum of two years of relevant experience in Mental Health or Substance Abuse is preferred.
- Ability to pass a background check as required by local authorities.
- Current CPR and First Aid certifications, along with an annual Tuberculosis screening.
- Capability to provide an Affidavit of Good Moral Character.
- Successful clearance in pre-employment drug testing and criminal record checks is necessary.
As part of your role at WestCare, you may be required to comply with new health and safety regulations, including vaccinations mandated by local, state, or federal authorities. Human Resources will provide updates regarding any such requirements.
