This program specifically serves youth aged 0-22 who have developmental, severe emotional, or physical challenges, ensuring they meet both functional and financial eligibility for Medicaid.

Role of the Support and Service Coordinator:

The Support and Service Coordinator (SSC) plays a crucial role in orchestrating and facilitating access to a comprehensive range of services and supports, tailored to the unique needs of children and their families. The SSC empowers families to achieve an inclusive and self-sufficient lifestyle by leveraging their expertise in available resources to develop personalized service plans that align with the family's goals. This role is pivotal in connecting families with community support and fostering their active participation in all aspects of community life. A primary focus of the SSC is to ensure the health, safety, and overall well-being of the child within their home and community.

Key Responsibilities:

  • Facilitate and coordinate services with families to address the developmental, emotional, and physical needs of enrolled youth.
  • Deliver family-centered, trauma-informed, and culturally competent services through regular communication and face-to-face interactions.
  • Promote community inclusion and access for youth and their families.
  • Assist families in establishing and maintaining eligibility for program services.
  • Conduct assessments and reassessments of the child's health, safety, and functional abilities.
  • Identify at-risk youth and ensure they receive appropriate care coordination.
  • Provide crisis intervention and resolution support as needed.
  • Fulfill mandated reporting duties regarding the well-being of children.
  • Develop and implement individualized service plans that cater to the specific needs of families.
  • Authorize and manage the provision of services through the CLTS Program.
  • Advocate for families to access necessary resources and support, regardless of funding sources.
  • Facilitate transitions related to CLTS Program enrollment and disenrollment.
  • Complete all required training and maintain accurate documentation of services provided.
  • Adhere to all policies and procedures established by the organization.

Additional Responsibilities:

  • Participate in training sessions and team meetings as required.
  • Provide coverage for colleagues as necessary.
  • Perform other job-related duties as assigned.

Qualifications:

Knowledge, Skills, and Abilities:

Demonstrated understanding of the needs of youth and young adults with disabilities; ability to manage challenging situations with patience and professionalism; proficiency in following both verbal and written instructions; capability to respond effectively in crisis scenarios; familiarity with Microsoft Office applications; efficient time management and multitasking skills; cultural sensitivity and awareness.

Minimum Requirements:

  • Bachelor's degree in a relevant health or human services field and at least one year of experience working with individuals with disabilities.
  • Completion of all mandatory training and shadowing programs.
  • Proficiency in computer applications, particularly Windows and Microsoft Word.
  • Ability to provide trauma-informed, family-centered, and culturally sensitive services.
  • Valid driver's license and reliable transportation.
  • Compliance with all employment prerequisites, including background checks.

Work Environment:

The role is primarily office-based, with responsibilities that include outreach to families and community engagement. Flexibility in scheduling is required, with some evening and weekend hours anticipated.

Physical Requirements:

Position may involve active engagement with youth, including potential physical intervention in critical situations. Driving is a necessary component of the role.
