Job Summary

Under the direction of the Comprehensive Community Services (CCS) Administrator, the Community Program Liaison will manage ongoing services to assure appropriate and quality care is provided to CCS consumers.

Key Responsibilities
  • Complete annual reviews of intake documentation to ensure compliance of DHS 36, Forward Health, and Milwaukee County policies and procedures.
  • Work closely with contracted providers to ensure their clinical, technical, and programmatic needs are met. Provide training and phone support as needed.
  • Develop, coordinate, and participate in measures to monitor quality of services provided by the CCS program, including audits and review of documentation to match Medicaid requirements.
  • Assist program leadership in organizing, attending, and running provider meetings which are held on both a routine and ad hoc basis for programmatic oversight, collaboration, and training.
  • Provide telephonic and walk-in support to clients and/or natural supports who are in the program or seeking to be in the program. Attend clinical staffing as well as meetings with other system partners who may also be serving our client base as needed.
  • Work closely with programmatic leadership to participate in Utilization Management/Utilization Review activities to ensure clients are receiving the right level of care, for the right amount of time, and are referred to other services and/or discharged as needed.
Requirements
  • Current resident of the State of Wisconsin at the time of application.
  • Bachelor's degree or higher in Psychology, Social Work, Human Services, or in a closely related field is required.
  • At least three (3) years of practical paid work experience in the Mental Health and/or Substance Abuse Disorder fields.
  • Proficient computer skills, including experience using Microsoft Office software such as Word and Excel.
