Job Overview

Position Objective:

The Housing Support Specialist plays a vital role in aiding individuals facing the threat of homelessness, including those who are chronically homeless or belong to homeless families. Within the framework of a street medicine initiative, this position is essential for client advocacy, outreach, case management, and facilitating access to benefits. The specialist serves as a key intermediary between clients and mental health/substance abuse services, ensuring they receive stable housing and necessary supportive services. By leveraging their expertise, they empower individuals to identify and overcome the unique challenges encountered on their journey to achieving and maintaining stable housing and overall wellness.

Main Responsibilities:

  • Engage in outreach efforts by collaborating with the street team to connect with and build relationships with individuals experiencing homelessness in various settings.
  • Establish trust and rapport while providing support to understand clients' needs and aspirations.
  • Conduct initial comprehensive assessments and develop intervention plans that address housing needs, service requirements, crisis intervention, and continuity of care.
  • Deliver case management and navigation services aimed at assisting clients and their families in securing and maintaining stable housing.
  • Support clients in completing housing applications, managing supportive and subsidized housing paperwork, exploring the rental market for affordable options, and advocating with potential landlords.
  • Collaborate with other agencies, coalitions, and participate in local community meetings.

Qualifications:

  • Associate's or Bachelor's Degree in Social Work, Psychology, Counseling, or a related discipline.
  • 1 to 2 years of experience in homeless services or case management, including housing navigation or community health work.
  • Familiarity with Housing Resources, HUD Guidelines, and various housing programs.
  • Excellent Communication and Relationship-Building Skills.
  • Proficient in Microsoft Word, Excel, and PowerPoint.
  • Understanding of Mental Health and Substance Abuse Services.
  • Skills in Crisis Intervention and Problem-Solving.
  • Ability to be Flexible and Adaptable.
