Job Description

Job Summary:

Titanium Healthcare is seeking a highly skilled Community Support Manager - Housing Specialist to join our team. As a key member of our community support team, you will be responsible for overseeing the housing services and ensuring that our members receive the support they need to achieve their goals.

Key Responsibilities:

  • Housing Services: Oversees the housing services provided to our members, including researching and establishing a list of available housing opportunities, developing partnerships with local providers, and ensuring that individualized housing plans are up-to-date with DHCS requirements.
  • Staff Supervision: Supervises and supports a team of community support staff, providing guidance and training to ensure that they are equipped to provide high-quality services to our members.
  • Member Support: Acts as a first point-of-contact for complex member issues, providing support and guidance to ensure that members receive the services they need.
  • Performance Monitoring: Monitors the performance of staff and identifies areas for improvement, implementing performance improvement plans as needed.
  • Community Engagement: Develops and maintains partnerships with local community organizations and providers to ensure that our members have access to the resources they need.

Requirements:

  • Education: 1-2 years' experience in healthcare, social work, public health, or a related field, or an Associate's Degree in healthcare.
  • Experience: 1-2 years' relevant experience in social work, case management, or working with unhoused and unsheltered individuals/families and veterans' programs.
  • Skills: Fluent in English (written and verbal), with preferred bilingual skills to address the Spanish-speaking population.
  • License/Certification: Current and valid Driver's License and proof of auto insurance.

Physical Requirements:

  • Office Work: Consistently operates a computer and other office equipment, with occasional movement around the office or community.
  • Driving: Ability to operate a vehicle and travel to meet with assigned members around the community, attend meetings, and visit different office locations as required.

Preferred Qualifications:

Bilingual: Bilingual skills to address the Spanish-speaking population.
