About the Role

Graham Windham is seeking a highly skilled Care Manager to join our Children's Health Home Care Management team. As a Care Manager, you will play a critical role in ensuring the continuity of care and comprehensive transitional care for our clients.

Key Responsibilities
  • Perform enrollment activities, including providing families with goals and obtaining necessary documents.
  • Complete The Child and Adolescent Needs and Strengths (CANS-NY) assessments every 6 months for children/youth on your caseload.
  • Conduct monthly required contacts based on level of care/need.
  • Coordinate between multidisciplinary teams to deliver health care services, including primary and specialized care.
  • Monitor changes in the client's condition that necessitates treatment and service changes.
  • Ensure that multidisciplinary team members are aware of admission and/or discharge from the Emergency Room, inpatient hospitalization, and/or residential care settings.
  • Complete Mental Health Coordination Unit admission, follow-up, and discharge forms for psychiatric hospitalizations within prescribed time frames.
  • Ensure that patients follow treatment recommendations and schedule follow-ups as needed.
  • Update health records in Connections.
  • Complete necessary documents for monthly care management billing to Health Home/Managed Care Organization.
Requirements
  • Bachelor's Degree in Health or Human Services.
  • Bilingual/Spanish.
  • New York State ID.
  • New York State Connections System and other data systems upon training.
  • Excellent communication, time management, and organizational skills.
