Job Summary

Morrison Healthcare seeks a skilled Community Support Specialist to provide vital community support services for team members in underserved areas. This role involves conducting needs assessments, analyzing data, and connecting team members with necessary resources, including housing, food, legal services, and mental health care.

The ideal candidate will possess excellent communication skills, be able to work independently, and have a strong understanding of social determinants of health. A Bachelor's degree in Health Education or a related field is required, with MSW or Certified Health Coach being preferred.

This position offers a competitive salary of $ $63,000 - $70,000 per year, depending on experience. In this role, you will:

  • Conduct needs assessments to understand social determinants of health and identify community resources.
  • Utilize Prisma Health's LiveWell platform to connect team members with necessary resources.
  • Act as a partner and ally for team members completing applications for assistance and benefit enrollment.
  • Liaise between team members and the emergency care fund to assist with financial approval for emergency circumstances.
  • Build and maintain relationships with community partners to provide optimal support for team members.
