Key Responsibilities

The Family Support Case Manager plays a crucial role in delivering case management services to families served by the Evanston Corps. This position operates under the supervision of the Regional Social Work Manager in collaboration with the Corps Officer.

Primary Duties

The Case Manager is tasked with ensuring the effective application of the Family Support model within the Evanston Corps.

  • Conducts intake and assessment services as outlined by the Family Support framework.
  • Provides a comprehensive suite of individualized services for families each year, which includes pre-screening, goal setting, ongoing case management, referrals, progress tracking, transition planning, and data reporting.
  • Maintains detailed case files for each client and engages in program development and educational initiatives.

Program Oversight and Evaluation

  • Ensures the accuracy of data and assists in program implementation as needed.
  • Participates in committees focused on case management and program evaluation.

Additional Responsibilities

  • Other duties as assigned.

Qualifications and Experience

  • A Bachelor's degree in a relevant human services field is required.
  • A minimum of two years of experience in casework or social services is essential.
  • Bilingual proficiency in English and Spanish is preferred.

Physical Requirements/Work Environment

  • Must be capable of lifting up to 25 lbs.
