Job Title: Family Youth Partner

Harmonium Inc. is seeking a dedicated and compassionate Family Youth Partner to join our team. As a key member of our Family Wellness Centers, you will play a vital role in providing care management, advocacy, education, outreach, mentoring, and coaching to K-8 students and their families.

Key Responsibilities:

  • Manage walk-ins and provide support services to students and families, including care management, advocacy, education, outreach, mentoring, and coaching.
  • Collaborate with the school's Care Team to provide parent engagement and outreach to identified at-risk students, supporting student interventions and family wellness.
  • Assist clients in achieving their wellness goals by providing support services, including continuity of treatment, groups, transition between programs, and linkages to other community resources.
  • Develop and implement youth-driven programs, including student clubs, workshops, and classroom presentations, to enhance students' lives.
  • Facilitate social-emotional, life skills, and behavioral health groups with students and families.
  • Participate in the billing structure for allowable services provided to students and families in the District.
  • Comply with confidentiality and privacy standards of protected health information and other applicable information and material in accordance with HIPAA and Harmonium policies and procedures.

Requirements:

  • Minimum of 2-year degree or combined equivalency of skills and demonstrated experience in the field of social services, human services, social work, counseling, case management, education, or the health services field.
  • Experience as a consumer and/or family member of someone who was a recipient of any public, community, education, and/or social service agency.
  • 2 years of experience in Case Management, Family Support, Advocacy, or related program preferred.
  • Experience with electronic documentation systems.
  • Bilingual Spanish highly preferred.

Harmonium Inc. offers a comprehensive benefits package, including medical, dental, vision, life insurance, long-term disability, and critical illness coverage, as well as additional voluntary options. We also offer a 403b Retirement Plan with a matching contribution of up to 2%, depending on years of service. Time off accrual for this position includes 8 hours of sick time per month and 10.66 hours of vacation per month for the first three years, increasing to 14 hours per month thereafter.

Starting Rate: $25.11 (all new hires begin at starting salary regardless of experience).
