Family Youth Partner

We are seeking a highly motivated and compassionate Family Youth Partner to join our team at Harmonium. As a key member of our Family Wellness Centers, you will play a vital role in managing K-8 student and family walk-ins, providing care management, advocacy, education, outreach, mentoring, and coaching.
As part of our school's Care Team, you will engage with parents and students to support student interventions and family wellness. Your expertise will help clients achieve their wellness goals by providing support services, including continuity of treatment, groups, transition between programs, and linkages to other community resources.
You will be a leader in developing wellness-driven behavioral health and wellness services and programs for students and families, which may include student clubs, student/parent workshops, and classroom presentations.
Key Responsibilities:
- Provide 1-on-1 support and share health education with students
- Implement youth-driven programs, including student clubs and advocacy
- Facilitate social emotional/life skills/behavioral health groups with students and families
- Engage with parents through outreach and home visits to support student behavior intervention
- Develop individualized client case plans
- Participate in the billing structure for allowable services provided to students and families
Requirements:
- Minimum of 2-year degree or combined equivalency of skills and demonstrated experience in social services, human services, social work, counseling, case management, education, or health services
- Experience as a consumer and/or family member of someone who received public, community, education, and/or social service agency services
- 2 years experience in Case Management, Family Support, Advocacy, or related program preferred
- Experience with electronic documentation systems
- Bilingual Spanish

- Time off accrual for this position includes 8 hours of sick time per month and 10.66 hours of vacation per month for the first three years, increasing to 14 hours per month thereafter
Starting Rate: $25.11
