Corewell Health South – Lakeland Hospitals is a comprehensive health system that provides a wide range of medical services to the community. Our system comprises three hospitals, an outpatient surgery center, a regional cancer center, rehabilitation centers, two long-term care residences, home care and hospice services, and 34 affiliate physician practice locations.
Scope of WorkIn this role, you will work collaboratively with the interdisciplinary team to assess, plan, implement, coordinate, monitor, and evaluate the strategies and services required to meet a patient's end-of-life care needs. You will create an outcome-based plan of care and continually adjust it through the utilization of the nursing process, relationship-centered care principles, and a holistic philosophy.
Our hospice team specializes in providing end-of-life care and symptom management to individuals with a life expectancy of six months or less. We serve patients in their homes, nursing facilities, and in the hospital. - Communicate effectively with staff, residents, and families to assess, develop, and maintain an individualized patient plan of care.
- Perform medication administration passes according to regulatory guidelines and inform MDS coordinators of significant resident/patient changes.
- Communicate with other members of the healthcare team to ensure continuity of care and coordination of services.
- Evaluate patient's and significant other's educational needs and develop and implement appropriate teaching strategies or make referrals.
- Observe and report patient's condition, needs, and outcomes of intervention to the physician and other team members.
- Teach and supervise the care provided by CNA, LPN, or other nursing service personnel according to regulatory and agency guidelines.
- Implement the plan of care by providing nursing services, including teaching, treatments, diagnostics, and preventive procedures.
- Participate in ongoing process improvement activities through the application of clinical paths, QA processes, and other related activities.
- Associate's Degree in Nursing is required.
- Bachelor's Degree in Nursing is preferred.
- 1 year of relevant experience in acute care, emergency, or rehab facility experience is preferred.
- Registered Nurse (RN) in the State of Michigan is required upon hire.
- Driver's License in the State of Michigan is required upon hire.
