Our health system comprises three hospitals, an outpatient surgery center, a regional cancer center, rehabilitation centers, two long-term care residences, home care and hospice services, and 34 affiliate physician practice locations. We provide both inpatient and outpatient medical services throughout Southwest Michigan.
Scope of WorkIn collaboration with the interdisciplinary team, assess, plan, implement, coordinate, monitor, and evaluate strategies and services required to meet a patient/family's end-of-life care needs. An outcome-based plan of care is created and continually adjusted through the nursing process, relationship-centered care principles, and a holistic philosophy, ensuring the changing needs of the patient/family are met while facilitating a symptom-free, peaceful, sacred death.
Spectrum Health Hospice specializes in providing end-of-life care and symptom management to individuals with a life expectancy of six months or less, serving individuals in their home, nursing facilities, and in the hospital. - Communicate effectively with staff, residents, and families, assessing, developing, and maintaining an individualized patient plan of care.
- Perform medication administration passes according to regulatory guidelines and inform MDS coordinators of significant resident/patient changes.
- Communicate with other healthcare team members to ensure continuity of care and coordination of services, acting as a patient/family advocate.
- Evaluate patient and significant other educational needs and develop and implement appropriate teaching strategies or make referrals.
- Observe and report patient conditions, needs, and outcomes of intervention to physicians and other team members, identifying problems and emergency situations, and initiating immediate 'life-saving' measures in the absence of a physician.
- Teach and supervise care provided by CNA, LPN, or other nursing service personnel according to regulatory and agency guidelines.
- Implement the plan of care by providing nursing services, including teaching, treatments, diagnostics, and preventive procedures, regularly evaluating patient progress toward established goals, recommending revisions to the plan as necessary, and planning for termination of service or discharge.
- Participate in ongoing process improvement activities through the application of clinical paths, QA processes, and other related activities, ensuring that processes and services are continuously monitored for quality, cost-effectiveness, and efficiency.
- Associate's Degree in Nursing – Required
- Bachelor's Degree in Nursing – Preferred
- 1 year of relevant experience in acute care, emergency, or rehab facility experience – Preferred
- Registered Nurse (RN) in the State of Michigan required upon hire
- Driver's License in the State of Michigan required upon hire
