Job Summary

We are seeking a skilled and compassionate RN Hospice Case Manager to join our team. As a RN Hospice Case Manager, you will be responsible for assessing, planning, implementing, and evaluating the care of patients with life-limiting illnesses. You will work collaboratively with the interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient and their family.

Key Responsibilities
  • Assess patients' physical, emotional, and spiritual needs and develop individualized care plans
  • Collaborate with the interdisciplinary team to coordinate care and services
  • Provide education and support to patients and their families
  • Monitor patients' progress and adjust care plans as needed
  • Communicate effectively with patients, families, and healthcare team members
Requirements
  • Associate's Degree in Nursing (ADN) or Bachelor's Degree in Nursing (BSN)
  • 1 year of experience in hospice care or a related field
  • Registered Nurse (RN) licensure in the State of Michigan
  • Valid driver's license in the State of Michigan
