Job SummaryWe are seeking a skilled and compassionate RN Case Manager to join our Hospice team. The successful candidate will be responsible for assessing, planning, implementing, and evaluating the care needs of patients with life-limiting illnesses. This is a full-time position working 40 hours per week, Monday to Friday.
Key Responsibilities- Assess the care needs of patients and develop individualized plans of care in collaboration with the interdisciplinary team.
- Coordinate and implement the plan of care, ensuring that patients receive high-quality, patient-centered care.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Monitor and evaluate the effectiveness of the plan of care, making adjustments as needed to ensure optimal patient outcomes.
- Participate in ongoing process improvement activities to enhance the quality and efficiency of care delivery.
- Associate's Degree in Nursing (ADN) or Bachelor's Degree in Nursing (BSN) required.
- 1 year of relevant experience in acute care, emergency, or rehab facility experience preferred.
- Registered Nurse (RN) licensure in the State of Michigan required.
- Driver's License in the State of Michigan required.
