Job SummaryWe are seeking a skilled and compassionate RN Case Manager to join our Hospice team. The successful candidate will be responsible for assessing, planning, implementing, and evaluating the care needs of patients with life-limiting illnesses. This is a full-time position working 40 hours per week, Monday through Friday.
Key Responsibilities- Assess the physical, emotional, and spiritual needs of patients and their families
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with the interdisciplinary team to ensure seamless care coordination
- Provide education and support to patients and their families to promote self-care and independence
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed
- Associate's Degree in Nursing (ADN) or Bachelor's Degree in Nursing (BSN)
- 1 year of relevant experience in acute care, emergency, or rehab facility experience
- Registered Nurse (RN) licensure in the State of Michigan
- Driver's License in the State of Michigan
