Corewell Health South – Lakeland Hospitals is a comprehensive healthcare system that provides a wide range of medical services to the community. Scope of WorkAs a Hospice Case Manager RN, you will work collaboratively with the interdisciplinary team to assess, plan, implement, coordinate, monitor, and evaluate the strategies and services required to meet a patient's end-of-life care needs. You will create an outcome-based plan of care and continually adjust it through the utilization of the nursing process, relationship-centered care principles, and a holistic philosophy. This ensures that the changing needs of the patient are met while facilitating a symptom-free, peaceful, sacred death.
Our Hospice ServicesSpectrum Health Hospice specializes in providing end-of-life care and symptom management to individuals with a life expectancy of six months or less. We serve patients in their homes, nursing facilities, and in the hospital. - Communicate effectively with staff, residents, and families.
- Assess, develop, and maintain an individualized patient plan of care with the patient.
- Perform medication administration passes according to regulatory guidelines.
- Communicate with other members of the healthcare team to ensure continuity of care and coordination of services.
- Evaluate patient's and significant other's educational needs and develop and implement appropriate teaching strategies or make appropriate referrals.
- Observe and report the patient's condition, needs, and outcomes of intervention to the physician and other team members.
- Identify problems and emergency situations and initiate immediate 'life-saving' measures in the absence of a physician.
- Teach and supervise the care provided by CNA, LPN, or other nursing service personnel according to regulatory and agency guidelines.
- Implement the plan of care by providing nursing services, including teaching, treatments, diagnostics, and preventive procedures.
- Participate in ongoing process improvement activities through the application of clinical paths, QA processes, and other related activities.
- Associate's Degree in Nursing – Required
- Bachelor's Degree in Nursing – Preferred
- 1 year of relevant experience in acute care, emergency, or rehab facility experience – Preferred
- Registered Nurse (RN) in the State of Michigan required upon hire
- Driver's License in the State of Michigan required upon hire
