Job Overview

Independent Living, Inc., a forward-thinking organization dedicated to cross-disability services, is in search of Community Support Specialists to enhance the lives of individuals in our care.

The Community Support Specialist will play a crucial role in delivering direct assistance to participants, fostering their integration into the community and promoting their ability to live independently. A high school diploma or equivalent is required, and we encourage individuals with disabilities to apply.

KEY RESPONSIBILITIES:

  • Safely transport participants, including those utilizing mobility aids, from their residences to community activities.
  • Implement the individualized Staff Action Plan developed collaboratively with the participant, their family, and the service planning team, focusing on achieving personal goals.
  • Facilitate participant engagement in various community activities, ensuring they work towards their established objectives.
  • Address any challenges that arise during daily activities with diplomacy and care.
  • Adapt activities to ensure all participants can engage to the fullest extent of their abilities.
  • Assist participants in reaching their goals as outlined in their community habilitation plans.
  • Provide feedback to the Employment & Family Services Director and Program Coordinator regarding participant goals for inclusion in their action plans.
  • Support participants in skill development and independence through community involvement.
  • Exhibit sensitivity to the personal needs of participants and their families, responding appropriately.
  • Utilize the AWARDS database for timely documentation of services and maintain accurate records as required by program regulations.
  • Complete all necessary training and participate in annual refresher courses as mandated by OPWDD and Independent Living.
  • Regularly meet with the Community Habilitation Coordinator and team members to ensure services align with participant needs and preferences.
  • Additional responsibilities and training may be assigned as necessary.

QUALIFICATIONS:

  • High school diploma or equivalent.
  • Preferred experience working with individuals with developmental disabilities.
  • Spanish language skills are a plus.
  • Valid NYS Driver License with a clean driving record and proof of current auto insurance.
  • Strong written and verbal communication skills.
  • Familiarity with community resources.
  • First Aid and CPR certification.
  • Dependability, flexibility, and a positive attitude are essential, along with strong organizational skills.
  • Commitment to honesty, respect for diversity, and high personal standards in alignment with the Independent Living philosophy.

WORK SCHEDULE:

  • Varies based on consumer needs.

COMPENSATION:

  • $16.00/hr - $21.08/hr
